MY Aging Care Services provides the following for older adults and their families.

I can help you REVIEW, ASSESS, NAVIGATE and PLAN:

Crisis Intervention

I provide crisis management by helping clients and their families navigate through emergency departments and hospitalizations, rehabilitation stays and ensuring that adequate care is available to the client during their  stay and as they transition. For families that live at a distance, I can be a much needed 24/7 emergency contact.

Advocacy

From physical problems to mental health and dementia related issues, I interact with the health care system effectively and frequently.  I will attend doctor appointments and facilitate communication between doctor, client and family.  These professionals help determine types of services (such as home health and hospice) that are right for a client and I assist in engaging and monitoring those services.

Local Resources

With a trusted network of professionals, I know the specifics of local resources in the community and how to access services. Local, cost-effective resourses are identified and engaged as needed.

Legal

Review powers of attorney, advanced care plans and have a network of trusted legal experts to review wills, estate and legacy plans.

Health and Disability

Understand current and future health care needs according to diagnoses and prognoses, family history and ensure you have the support and services to remain as healthy as possible as you age. Understand what help is available and what it costs.

Financial

Review current financial position, knowing the value of your assets and living expenses. Understand potential expenses related to your aging needs and priorities.

Housing

Aging in Place – I promote autonomy and independence to the extent that it is possible and respect the desire to age in place while remaining safe and secure. Organizing, downsizing, maintenance, assistive devices and use of technology can all be addressed.

I help families and clients evaluate and select appropriate level of housing or residential options that their aging needs and vision.

Family

I help families adjust, cope and problem-solve around long-distance and in-home caregiving, addressing care concerns, internal conflicts and differences of opinion about long-term care planning.
